Community Action Team Case Manager

Job Description

Institute for Child and Family Health (ICFH) is a not-for-profit established in 1945 that provides behavioral health and prevention services to children and their families in Miami-Dade, Broward, and Palm Beach counties. It is one of the oldest and largest community mental health centers exclusively dedicated to the well-being of children and their families in the State of Florida. ICFH employs more than 250 employees, and administers multiple contracts and grants from various funding sources to provide services in the following six areas: Child and Family Counseling; Substance Abuse Prevention; Early Childhood Programs; Violence Prevention; Literacy and Educational Development; and Professional Training Programs.




The CAT Case Manager is responsible for providing community-based case management services to assigned clients.  These services include, but are not limited to assessing clients’ needs, advocating and linking to services and monitoring of services.  All case management services must be provided in accordance with funder requirements, ICFH policies and procedures and all applicable laws and regulations. 


The CAT Case Manager: Coordinates services for assigned clients. Advocates on behalf of the families. Provides access to a variety of services and supports, including but not limited to: primary medical and dental care; basic needs such as housing and transportation; tutoring and educational services; employment and vocational services; legal services; and other behavioral health services as needed. Completes assessments for all assigned clients. Completes service plans and service plan reviews for all assigned clients.Completes documentation that is accurate, thorough, individualized, and complete for each client and submitted within given deadlines.Works in conjunction with treatment team and maintains consistent communication with treatment team members.Coordinates and participates in treatment team meetings.

Job Requirements



A Bachelor’s degree with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related field which includes the study of human behavior and development; and a minimum of one year of experience working with children and adolescents with serious emotional disturbances; or a Bachelor’s Degree with a major in another field and a minimum of three years of experience working with children or adolescents with serious emotional disturbances. In addition, candidates must be able to provide in-home services and work evening hours. Required trainings must be completed after hire. Bilingual (English/Spanish) a plus.
